PARK GUIDELINES
- Visiting hours for the Columbary are from 7:00AM to 6:00PM daily.
- The Columbary is open 24-hours during October 31 and November 1.
- The maintenance crew will remove all ritual instruments and ceremonial
offerings (i.e. flowers, candles, incense sticks, etc.) the following day.
- Food & drinks and music are allowed provided that such acts do not
obstruct the pathways of the Columbary, especially the memorial ceremonies and
rituals of other visitors and guests.
- Littering is strictly prohibited within the premises of the Columbary.
Food remains and other waste materials must be removed, cleaned and placed in
the proper receptacles. Apparatuses and utensils, including radio equipments,
and other items left in the area will be disposed if not reclaimed the
following day.
- Visitors and guests are advised to secure their personal belongings. The
Columbary will not be responsible for lost items within its premises. Children
must be accompanied by competent adults at all times.
- The bringing and consuming of alcoholic beverages within the premises of
the Columbary will not be tolerated. Firearms and other deadly weapons are not
welcome inside the premises and will be secured by security officers prior to
entry.
- Security officers can be found patrolling the premises of the Columbary at
all times and may render assistance or disciplinary action if so needed.
Security officers will not hesitate to remove and/or ban visitors and guests
who continuously violate the Park's Rules and Regulations.
- Unruly and abusive behavior (i.e. vandalism, public scandal, intoxication,
etc.) will subject violators to the apprehension of police officers and law
enforcement.
- Being a Holy Ground, The Columbary's solemnity will be observed at all
times.
INTERMENT REQUESTS AND SPECIAL ARRANGEMENTS
- Interment requests will be allowed with a Certificate of Ownership,
subject to the Interment Requirements and Procedures of the Columbary. All
requirements must be submitted three (3) working days prior to the Interment
proper.
- Interment Fee must be paid before any Interment takes place. Fee is based
per Urn, per Interment and does not include Stainless Inscription on granite.
- Columbary owners may apply for change of unit/location provided that no
Interment has been made on the Columbarium Unit, and is subject to the
Guidelines on Changing the Ownership/Location of Columbarium Unit of the
Columbary. Once an Interment has taken place, it may not be relocated to
another area within the Columbary.
- Special arrangements (i.e. use of Multipurpose Hall for masses, gatherings
that may use substantial floor area, tables & chairs, flowers, candles,
offertory items, etc.) must be submitted through a Letter of Request addressed
to the Administrator of the Columbary five (5) working days prior to the
Interment proper or event, and is subject for approval by the Administration.
Further fees may apply.
- Memorial and/or cremation services purchased via pre-need or at-need may
be used in conjunction with the Interment on the Columbarium Unit. Other
services such as transportation requests, if not included with the package
from the provider, may be entertained though a Letter of Request five (5)
working days prior to the Interment proper. Further fees may apply.
- Urns may be borrowed and brought home provided that a Letter of Request
from the Owner or the legal heirs is submitted three (3) working days prior to
the event. A Resealing Fee will be charged upon the return of the Urn.

GUIDELINES ON CHANGING THE OWNERSHIP OR UNIT/LOCATION OF COLUMBARIUM UNIT
- All Columbarium Units are transferable, assignable, changeable, and sellable to another person or entity.
- Any changes, either for the ownership or unit/location of the Columbary, the following must be complied:
- The new owner, transferee, assignee, or buyer must agree to the Offer to Reserve/Purchase signed by the original owner. The new owner must also sign a new Offer to Reserve/Purchase.
- If a sale transaction, the seller must execute a Deed of Sale in favor of the buyer. Government taxes, if any, are for the account of the buyer. The previous owner must surrender the old Certificate of Ownership to allow the issuance of a new Certificate of Ownership under the name of the new owner.
- If transaction is an assignment, the owner must execute a Deed of Assignment in favor of the assignee.
- If changing the unit/location of the Columbarium Unit, the owner must agree to pay the additional amount of the new unit’s current price against the previous purchase price. If changing the unit/location is a downgrade on the value of the original price, no refund shall be made.
- If the Columbary Vault is being paid under installment plan, the new owner must agree to assume the balance and issue the corresponding post-dated checks (PDCs) for the remaining amount. No transfer, assignment or change of unit/location will be allowed until the installment payments as scheduled are updated up to the next due date and/or redeemed from default.
- No Interment has been made on the Columbarium Unit.
- Payment of Service Fee of P2,000.00. (Fee for the first instance of change of location is waived.)
- All marketing commissions, if any, cease after the change of ownership of Columbarium Unit.

INTERMENT REQUIREMENTS AND PROCEDURES
- Original copy of the Certificate of Ownership (COO) issued by Columbary of the Divine Mercy Shrine.
- Certified true copy of the Death Certificate officially validated by the city registrar.
- Valid I.D. (with signature and picture) of the certificate holder.
- Notarized Deed of Assignment by the registered owner if assigning the Columbary Vault to another person and payment of Assignment Fee of P2,000.00 if the assignee is not an immediate family member of the registered owner.
- If the owner is not available, a duly executed Letter of Authority is required.
- If the owner is outside the Philippines, a duly executed Special Power of Attorney is required.
- If the owner is a group/entity, a duly executed Secretary Certificate & Board Resolution is required.
- Payment of Interment Fee of P3,500.00 (per Urn, per Interment).
- Inscription (Computer Sandblasting) on granite is P85.00 per alphanumeric, accent or symbolic character.
> All requirements must be submitted to Columbary of the Divine Mercy Shrine Main Office three (3) working days prior to the Interment proper.
> Any Special Arrangements must be submitted through a Letter of Request addressed to the Administrator of Columbary of the Divine Mercy Shrine five (5) working days prior to the Interment proper and is subject for approval by the Administration.
> Inscription Fee must be paid seven (7) days prior to the Interment proper as allotment for the preparation.
